There's nothing I can say that the other 135000+ positive reviews haven't already. For me, this is the closest to a perfect video game that I have ever experienced - and possibly ever will. It is the only game I have ever played where I feel like I didn't pay enough.

Quality 94/100 AAA - Developed by CD Projekt Red
Performance 95/100 Steady 60fps in 1080p ultra, well optimised given the scale.
Utility 95/100 Uses Steam Cloud, both manual and auto-saving.
Controls 90/100 Good for the most part, occasionally a little wonky.
Gameplay 96/100 Incredibly detailed large open-world design with plenty to do.
Content 95/100 Copious amounts of content across the entire span of the world.
Immersion 98/100 Incredible atmosphere throughout with an incredible soundtrack to top it off.
Challenge 93/100 Multiple difficulty settings, as well as enemy upscaling for the extra challenge.
Design 97/100 Consistently fantastic design throughout.
Visuals/Level 100/100 Incredible world and character details with a variety of biomes
Audio 100/100 Arguably the greatest soundtrack I've ever heard from a video game.
Story 97/100 Unbelievably written story. 36 possible endings.
UI 91/100 Slightly messy at first but easier to understand after a few hours of playing.
Overall 97/100 As close to perfect as I may ever experience in video game form.

What's so great about this game exactly?
You can tell that CD Projekt Red poured their heart and soul into this game, and that is unfortunately something that the video game industry has been lacking as of late. The world is intricately crafted and it feels alive. There are different biomes which genuinely feel like completely different places and regional accents to fit. The attention to detail is next to none.

The character development is unbelievable, never have I cared so much about what happens to a character in a video game than I have in this one. You almost become attached to certain characters and it's an odd feeling to say the least.

Assuming you're not rushing through the game going quest to quest, you can easily get 150-200 hours of enjoyment out of this game.
