I think it's the minimap's fault. For now you can uncheck the 'Allow minimap' checkbox on the HUD options.

Just tried both the Workshop and GitHub versions. Couldn't replicate it. I kill myself, and the death screen appears without a problem, then I respawn and there's no extra grey window. Can you post a screenshot with your HUD configuration? Have you checked if it could be another addon?

A grey window simply called "Label" stays on the screen indefinitely. This is the game over HUD for when you die for the cry of fear hud, as I found out via killing my own player via the "kill" console command.
It is the only interactive and persists after death.

Important problem with the cry of fear hud! I think we should adress it ASAP.
The bug in question BREAKS Gmod in its entirety and is malicious.

You can put this line of code inside your gamemode to always show it when loaded:
