Unforgiving gameplay paired with methodical combat and game mechanics that need to be learned rather than understood. A paralel or three drawn to Dark Souls (or any of the Souls series for that matter) is very accurate. While most might be put off by that last comment, there's a serious amount of satisfaction to reaching the next area, without the feeling of self-flagellation lingering over the monumental sum of hours and days you'll spend trying to beat this game.

Also, humanoid penguins dualwielding swords and rocking pirate hats. It's the little things.
