Nightmare difficulty isn't difficult, it's cheap. Try doing a single mission? Expect 3 Ferals, 2 Juggernauts, 5 Bloaters, 4 hordes, and 9 screamers to spawn at once. I got bored of the ridiculous amount of effort and planning a simple fetch quest took because the game literally threw the population of a large town at you every time they wanted you to do a single mission. And killing Plague hearts are almost impossible since twice those numbers just crawl out of the walls.
