Literally didn't know what R key is for before this dlc, now using it every run.
Game is heavily based on luck since this extension. A lot of good items were nerfed (I don't even take tech-x anymore, since it just tend to make the run much worse) or their abilities completely erased, meanwhile some of the new ones are literally gamebreaking (revelation, c-section, rock bottom, lemegeton, recently nerfed psy fly). The whole idea behind: "making every item equal, so game can be played with anything" is really not down to earth, since you still have items like lemon mishap, cain's other eye or bean items which can be effective only in very specific synergies, mostly just useless.
The angel room mechanic is really confusing, since sometimes without getting hit even once it can spawn on the last possible floor if at all.
Some mini-bosses and enemies from antibirth spawn bullethell, you can certainly tell which ones are made by Edmund and which by modders. It really emphasizes the "take the original enemy and add 3x bullets" idea, instead of coming up with new types of attacks. There are room layouts that can instadamage you no matter how skilled you are, literally not giving you any chance to dodge.
Also Delirium and Super Greed mode are still the most unenjoyable things in this game.
The dlc definitely adds a lot of content and fun; The Mother and Dogma are definitely the best bosses in the game. They have fantastic, greatly announced attacks, even if hard they're totally possible to memorise and eventually learn, great music themes and general themes. Tainted characters are very creative and enjoyable, sometimes more than the basic characters. Sadly, a lot of changes make it just not as light and fun to play as before.
