Been able to get all achievements since the patch except this one, good work on the update, but how do i get this one? I've done what it states multiple times to no avail, I believe it unlocked on the wrong level in previous build but even that doesn't unlock anything now. Any pointers? Or is it just broken?...

I unlocked this achievement before even completing Chapter 1 - possibly after level 3 or 4 of the chapter. I hadn't died in that specific level, but I had died in the previous one.
