How to unlock the achievement - One Sword to Slay Them All

This is a very brief guide that explains what is required to obtain Ultima Weapon.

Fix Princess Sara's Pendant

Speak to a Mognet Moogle as Ingus until you have read the letter "Help!" from Princess Sara.
Speak to a Mognet Moogle as Refia until you have read the letter "Legendary Smith" from Takka.
Talk to Sara in Castle Sasune, East Tower.
Talk to Takka in Kazus.
Talk to the Legendary Smith in Northwestern Saronia, she is on the right of the area.
Return the fixed Pendant to Princess Sara.

Defeat the Monster in Cid's Basement

Speak to a Mognet Moogle as Luneth until you have read the letter "Something in the Basement..." from Cid.
Talk to Cid in Canaan.
Deafet the Monster "Aeon" in Cid's Basement. Not difficult it has 10000 HP and casts Quake when its HP is low.
Talk to Cid again for him to identify the "Unkown Metal" as Orichalcum.

Open the Door to Eureka

Once you have entered the Crystal Tower and opened the Door to Eureka, return through the Ancient Maze. To open the door just head north from the entrance.
Talk to the Legendary Smith in Falgabard. Just right of the weapon shop. She will now create for you "Ultima Weapon"

Falgabard is the town just South West of Saronia. You will need to use the Invincible to get there.

note: I did not find the Mognet quests until I had the Invincible so I am not sure when they actually unlock. But as you cannot complete the quest chain until you have opened the door to Eureka you can just start it then.

Update: Changed the Screenshot to indicate Falgabard.

I was able to get the Help! letter from Sara after having a certain amount of gil on me. Not sure how much, but between 10-600k.

AFTER you got the 1k Kills you have to go back as the Girl and Talk to your Father to UNLOCK the OCHICHALKUM LATTER and the SOMETHING IN THE BASEMENT LETTER!!! ARG!!!

The strange thing is, I was at 638 enemies slain, and when I grinded up to 1000, the letter unlocked.
So that worked for me, thank you! I cna finally get the Ultima Weapon haha

Just for future commenters you DON'T need 1000 killed, I got the 5th Takka letter at around 650 kills before eureka (bestiary 70-72% complete)

yeah guys read the comments below: to get the Orichalcum letter from Takka you need 1000 monsters killed!! if you are having this problem!

after u kill 1000 enemies you get the legendary smith letter from takka as refia moogle mail net.

i noticed you have to use the key's that ulia and doga gave you to open eureka door and xanxune darkness door in crystal tower before you get letter "Help!" from princess sara as ingus, after u do that u get the letter help from sarah so you can continue the quest to get ultima weapon, just thought id help.

I want to make clear the conditions for getting the sword IF the guide is not working for you.

Follow the guide up until you probably are not getting Cid's basement letter (his last one).

Kill 1000 enemies. You can check your progress under the Achievements either at Gysahl (go to the Inn, there's a hidden path on the right side leading to a person) or at a Moogle in the Ultimate airship. Once the total number of enemies has hit 1000, you'll get Takka's 5th letter called "Orichalcum." Read it, back out, and you'll see Cid has sent you the letter. NOW you can proceed.
