Story-related achievements
The following achievements are all story-related, thus cannot be missed.
They will all be acquired during your first playthrough and won't be discussed here:

Save a lot!
You can save & reload anytime you want, use this to your advantage.

 
Useful second playthrough trick
As in previous Blackwell games, use your numeric keypad to speed up your 2nd playthrough:
  • Keep any key pressed to skip dialogs and comments (I use 0)
  • Use 1-9 to quickly choose a question/answer when the game prompts you
 
 
Step 1: First playthrough
  Blowey Mallone
  Blow on everyone you can find

Use Joey's blow on every living character (ghosts don't count) you encounter.

  2. Detective Durkin (Police station)
  3. Receptionist (Police station) aka Cory Palmer
  4. Receptionist (George Ostin)
  5. Emil Haskins
  6. Clerck (Abacus)
  7. Father Ullman (Grace Church)
  8. Runner (Field's Gym)
  9. Jim Pebbles (Channel 11)
  10. Maggie Fielding (Peter's appartment, Greenwood Cemetery)
  11. Bartender (Vantini's)
  12. Ray (Vantini's)
  13. Doctor Quentin (Psychiatric hospital)
  14. Receptionist (Psychiatric hospital)
  • Blow on them as soon as you're able to, otherwise you might miss someone.
  • It doesn't matter if Joey actually blows on them or refuses: you just have to try on each one.
  • As far as I know, there's no way to blow on both Lia and Father Michael Cooper.
  • The hospital receptionist is probably not needed (got the achievement with Doctor Quentin).

 
  Snow Plow
  Somebody's got to do it...
  ●●● Highly missable

Make Rosa walk back and forth over the snow in every snowy areas until Joey gets annoyed.
  1. Karth House (opening area)
  2. Police station (after interrogation, when Detective Durkin will be gone)
  3. Grace Church
  4. Lia Pierro's house
  5. Exeter House
  6. George Ostin's roof patio
  7. Grace School
  8. Field's Gym
  9. Heather's old address
  10. Vantini's
  11. Vantini's backyard
  12. Greenwood Cemetery
  13. Grace School roof
  • Annoy Joey as soon as you enter a new snowy area, otherwise you might miss one
  • The achievement should unlock after any 12 areas, so you can miss one and still get it (try not to though). Rosa will reply she walks in order to keep herself warm.

 
  Deception
  Try to lie your way out of it
  ●●● Highly missable

Just lie (or avoid telling the truth) during the whole game.
  1. Store Clerk - Yes, you have Permission.
  2. Emil Haskins - Choose dialogue options that avoid things you are sure of.
  3. Kendra - About Trollgate.
  4. Lia - Kendra's soul is safe, BEFORE you escort her to Emil's.
  5. Interrogation - 1. Say Lia was dead when you got there.
  6. Interrogation - 2. Lie about Permission for the office.
  7. Ray (Vantini's) - Say you were looking for the bathroom.
  • Some dialogue sequences lock after you choose 1 thing to say, so SAVE in case you mess up!
  • For some reason, this achievement will only unlock once you've finished the game.

  Goldfarb Digger
  Find a secret room
  ●○○ Can be done in any playthrough

  • Location: Vantini's

  • Use Joey's tie on Abe Goldfarb (Joey's voice actor) portrait on the wall.
  • If you don't know how it looks... just look at the achievement icon ;)

 
  Dumpster Jumper
  Make a leap without falling once
  ●○○ Can be done in any playthrough

  • Location: Vantini's backyard

  • Grab the rag
  • Use your cup of coffee, then the rag on the dumpster
  • Climb on it and click on the ladder (any choice should work)

 
  Epiphany
  The hows and whys of Joey
  ●○○ Can be done in any playthrough

  • Location: Grace School

  • Don't use the tie on Rosa as soon as she's possessed
  • Talk with her first, until the achievement pops up

 
  Laying it Bear
  Learn a long kept secret
  ●○○ Can be done in any playthrough

  • Location: The void

  • Convince Aunt Lauren to get up
  • Talk with her until the achievement pops up
 
Step 2: Second playthrough
  Move on and Listen
  Finish the game in commentary mode (from the first screen till the last)
  ●○○ Missable

  • Before starting your 2nd playthrough, click the Options button in the title screen
  • Activate Commentary mode
  • Go back and start a new game

  • Don't ever deactivate the comments until the end! You can press a key to skip them.
  • The achievement will unlock once you've finished the game.

 
  Softly Softly
  Create no more than 450 footprints
  ●●● Highly missable

  • Use Joey to explore: do not move Rosa until it's really needed!
  • Use Joey to exit screens: place him as close as possible to the exit, then click it. When Rosa will try to follow him, she'll only have enough time to make 1 or 2 steps.
  • Use Joey to call Rosa: whenever you can, explore with Joey, then call Rosa in the room you need her.
  • Save often and reload if you find a way to walk less.
  • Use your tie on possessed Rosa as soon as possible.

  • Most people on forums talk about 300 steps rather than 450. Seems like the game as been updated since then, but aim for less than 300 just to be sure, it isn't that hard.
  • The achievement will unlock once you've finished the game.

 
  Ghostly silent
  Keep clam and keep cool
  ●●○ Opposite achievement in 1st playthrough (Deception)

  • Location: Interrogation room

  • Just keep silent whenever you can during the whole interrogation.
  • The achievement will unlock as soon as you exit the room.

For the steps to unlock the "Deception" Achievement, the guide says to tell a lie to Emil. However there is no option to tell him that is a lie, or we know as a lie as all the options we can choose from to tell him are all true facts.

When I saved Mary I did tell her I didn't know what happened to the others in the Karth House as she walked towards the light which is obviously a lie.

@redrobin I don't think you missed too much. Joey was tha last ghost they wanted to save before the Countess banished Madeline , but I think this was revealed at an other point of the story.

Just finished my first playthrough. Does anyone know what the dialogue options that lead to Epiphany (The hows and whys of Joey) reveal? I just used the tie straightaway and now I want to know if the dialogue gave any more info on Joey's history, like how he became a spirit guide from a ghost Madeline and Countess were going to help in the flashbacks. I feel like I missed something in that story.

Just a quick note that I got the Blowey Mallone achievement without blowing on Emil - you never actually get the opportunity to do so when controlling Joey. The achievement popped when I blew on the receptionist at the asylum.

I couldnt get the chance to blow on Emil Haskins as Joey, not sure if I mucked it up. Will see when I finish :)

Update for Deception: I got the achievement and I stayed silent during the entire interrogation. So I got Ghostly Silent and Deception on the same playthrough

So I just finished getting all of the achievements and I think that you need to update Snow Plow. I found another guide that mentioned that there are two places where you can also get a reaction from someone OTHER than Joey: Police Station with Durkin, and outside Grace School with Madeline. I ran through the game twice using only your list and the achievement didn't pop, the third time I added these in and it popped out back of Vantini. Also I could not get Joey (or anyone else) to react on top of Grace School at the end of the game.

I think some achievements are tied to the game whole game, and not to a save. I did a lot of reloads to read all possible dialogs and got Deception in the end. And I'm sure I didn't keep the saves I lied.
I'm going to try this with Snow Plow. Reload every nearest save to the plots to walk on them and see if the achievement pops up.
