To unlock hard mode without beating the game first, go to the main menu and put in this sequence of inputs quickly (See below)

XBOX 360 Controller

Left, Right, Left, Right, X, Y, X, X, Back

KEYBOARD

Left arrow, Right arrow, Left arrow, Right arrow, A, W, A, A, Space

You should hear the sound of a pinata popping, then if you start a new game, you'll be allowed to select Hard Mode.

Hard Mode code works, probably better to do it on KB than on a controller

If you have unlocked it, it shouldn't make an activation sound and its probably already unlocked and you have to start a new game to select it.

I used a wireless 360 controller, BUT I also tried with keyboard. What is going on?

That is exactly where i entered it. What type of gamepad do you use exactly model and wired or wireless?

It can be unlocked even if you even already started a game.
