There's a cube to grab, before using it to exit. Some momentums to proceed.
Note : A bit of dexterity is required.

The way to exit is inspired by a room from AmnDragon's Cubi.

@kimist, I made a fresh update enlarging the area, to let more time for the execution.
Your solution is very similar to the intended : launch the cube, then you a short time after, then portal its panel, then yours. Thanks for testing my workshop !

Well, with your update, the solution of toncica and jgaultney were not doable. Not sure if I did the intended. My solution used some fast portal shooting. Placed portal on floor through little hole right in front of the faithplate. Other portal on central slanted surface. Launched cube. Then, while cube is going up through floor portal, launched me. Then while in air, shot portals to get cube to funnel and then me to exit.
